Abstract

medication errors in the stages of prescribing, transcribing, dispensing, and administering prescriptions. Incompleteness of a prescription can cause medication errors in the prescription. This researching is a descriptive.observational study aims to determine the number in medication errors of the prescribing phase of hypertension patients and determine the type of medication errors in the prescribing phase at Sukorame..Public Health Center. Based on the results of the study, it was found that the incidence of medication error was 39% or 69 sheets of the 175 prescriptions of hypertension patients studied. The percentage of incidents of medication errors in..administrative aspects that did not include doctor's SIP was 39%, doctor's name was 39% and patient's BB was 100%. In the pharmaceutical aspect, there were 33% medicationerrors and 15% did not include the strength of the drug preparation. In the clinical aspect, there is no medication error or as much as 0%. Based on the research in this study, it can be concluded that of incidence of medication error in the prescribing phase still occurs in the prescription of hypertension patients at Sukorame Public Health Center in 2019.
